Özet

Introduction: We evaluated the outcome of dorsal midline tunica albuginea plication in children with ventral penile curvatures at the Department of Urology. Materials and Methods: Eight patients with ventral penile curvature underwent dorsal midline tunica albuginea plication with or without concomitant hypospadias repairs between 2004 and 2006. None of the patients had undergone penile surgeries previously. One patient had chordee without hypospadias and the others had meatal positions varying in between coronal and penoscrotal regions. TIPU, Thiersch-Duplay with Onlay Island Flap, urethral mobilization, and one-stage dorsal island tube flap repairs were performed to cure hypospadias. A residual curvature of 30 degrees or more with artificial erection after penile degloving and chordee excision was our indication for plication that was achieved at the opposite site of the apex of the curvature with a single prolene suture knot. No incisions or excisions were made on the tunica albuginea. Results: Mean age and median follow-up time were 30.1 (9-60) and 27.5 (12-32) months respectively. In one case, intraoperative bleeding due to dorsal venous injury by the needle caused no postoperative hematoma and healing was uneventful. No major complications occurred during surgery and follow-up, and the cosmetic outcome of orthoplasty was excellent. Conclusion: Dorsal tunica albuginea plication is a safe and effective procedure in children with ventral penile curvatures, yet the functional outcome after puberty needs to be studied.
