Özet
Piriformis syndrome is a form of sciatica that is caused by compression of the sciatic nerve at the level of the ischial tuberosity. The condition was first described in 1928 and the "piriformis syndrome" term was first suggested in 1947. Piriformis syndrome commonly occurs in middle-aged patients, with females constituting most of the reported cases. Piriformis syndrome is mostly caused by trauma to the buttocks and the following soft tissue inflammation. Because of the inflammation, muscle spasm occurs and compresses the sciatic nerve. Patients often describe chronic buttock pain and pain when getting up from a sitting position. The diagnosis of piriformis syndrome is based on clinical signs and symptoms, as agreed upon clinical criteria have not yet been defined. Special physical examination tests can also be performed to clarify the etiology of piriformis syndrome. Piriformis syndrome treatment consists of different approaches such as conservative management, medical management, physical therapy, steroid injections, botulinum injections, and surgical techniques. Currently, it is suggested that patients start with conservative treatment, and if symptoms persist; treatment should progress to more invasive approaches. Further studies are needed to define structured diagnostic criteria for piriformis syndrome and to clarify the potency of newer therapy methods.
